The question about what is cosmetic dentistry? Well, I guess the simple term is whatever is going to make you better or look better. Is enhancement of the look of the teeth and eventually the smile. That could range from anything nowadays, even facial injectables, wrinkle relaxants that are around a small area – are part of it. So we are moving not only from the inside of the mouth but also to the outside of the mouth and the face because it’s in the close relationship to that. If we’re talking about inside the mouth that could range from anything from your whitening solution to veneers, crowns, bridges, implants if it’s in the front area. They’re all cosmetic procedures. A lot to people ask about what our dental implants. Well, think of it like that, You have an empty space in the mouth and that empty space used to be occupied by the root of the tooth. Now, the root of the tooth is the part that we don’t see, it’s inside the bone. It’s holding what we see as a tooth on the outside. Now, if that is lost then I have no foundation to build anything in there. So an implant is basic replacement of that root. Now in layman’s term basically, it’s a titanium screw. Now, it’s not that size. But it looks like that. And the idea is that goes in the bone. And once you put that in there and leave it there for a period of time, then the bone will grow around that material and once it’s fixed in the bone, it has no way to go. On the inside of it, there is what we call an internal screw. Now that internal screw is what’s going to be exposed through the gum. So that is the platform. The root, if you like. Or the artificial one. And then on top of that, you can put whatever you like. It could be a part of a bridge. It could be a support for a denture or it could be just a support for one single tooth and then you can put an attachment there, then you will have a little screw to fixate that together and that could become a tooth. A lot of people ask about the actual procedure when we do implants. Now here are just SMILE we use NobleClinician Software, which basically is a 3D imaging software. What we normally do and a lot of our existing patients Know this, that when we are ready to actually prepare for the treatment planning for an implant, we send the patient for a c.t. scan of that particular area, that comes into us then would be fed into the software. And from then on that recreate the 3D image of the skull of that particular part. And then what I normally do is just go through their measure and decide what kind of implant we put in there. What angle, what length and literally doing what we call a kind of digital surgery if you like. And once we’re happy with everything we move on to the iPad where I can take it with me, show the patient at the separate appointments. This is what we have to do. And because of such and when we have an agreement that Ipad, believe it or not, will actually control the device that puts the implants in as well. And the whole aim is not only to impress a patient, But to actually make sure that we are that accurate and that delicate about the procedure that we are providing. So we end up with the best product that we can get. On the screen here, what you can see is a 3D image of an implant site. And here, I’ve already placed an implant there But you could imagine that we had nothing in that area here. So what I normally do as you can see, the slide will move and will show me particular sides of that 3D image. And here you can see the tooth in the bone itself. And when we move on into an empty space. It will look like that. So that empty space here. We don’t know from the outside. How wide, how long, in this particular area, you can see the sinus. These are all important parts of anatomy that we have to measure and know about before we can go ahead with any implant placement. Now when we move on forward. You can see here the implant or cross-section of an implant that we will put in. So I know exactly how long that implant, how wide it is, Where is it exactly. If I don’t like I can simply just move on and change things around. And you can just choose the optimum placement, then that could be transferred to the patient’s mouth. So we know I’m not going to go this way. I have to go that way. And that angle is measured. And once you do that, you can transfer that onto the iPad and on the iPad. I can do this for demonstration of what we can do to the patient as well as actually using the iPad to control what we call the micro motor that places the implant in there and it can simply go ahead and choose on to go like that. And it can show the patient almost like an X-ray vision of the area. And I can move on to which area that I can see. And then I can just go ahead and click on that and show the patients where the implant is going to sit and exactly at what angle. And if there are any limitations. What are they? Or precautions and that will make it easier for both of us to understand what we have to do. And from then on when we have an agreement we move on into actually doing the procedure itself. Well, as you can see, the machine is here. All that it is, It’s a computer controlled light. It’s a special blue light that activates the Zoom gel, which is basically a bleaching gel. Now the difference between this machine and your normal whitening gel is this – Once you start mixing a whitening gel together the active ingredients will become active for about two to three minutes. And from then on, It’s nothing. With this, Zoom people invented the way of making sure that the activation of the gel is happening over with the light. So it’s what they call a sustained release of the bleaching solution. So rather than we’re having a big hit of the whitening solution for two minutes. I can control the release on a steady basis for 15 minutes. And the idea is that we continuing the whitening procedure at the same level until that finishes. How we do it? Basically what we’d like to do is get the patients in the chair and isolate everything. Whatever is not teeth has to be covered because of the power of the bleaching solution. So we spent about a good 15 minutes making sure that the gum is covered, the cheeks, the lips, everything is protected. Once we do that, then we’ll apply our first layer of the Zoom whitening gel. The light comes in and clips into a mouth probe and the 15 minute cycle starts. Once that finishes, it will shut down automatically. We come in wipe the old gel, introduce another one And we do that three times. And by the end of the 45 minute session, you should move at least six shades lighter. So one hour crowns. We use the E4D system, which is basically a laser scanner that scans the tooth after we finish prepping it. That comes on a computer. And then on there, we decide what design do we choose for that tooth. Once we're happy with that. That will get wirelessly transmitted to the milling machine. All you do is put a block of porcelain in there and about 20 minutes down or so you will end up having a shell of a tooth that sits on a broken tooth and fixed the problem on the spot. Now traditionally with the crowns we normally do the preparation of a tooth, we take impressions then we put a temporary crown on the area. Impressions goes to a nominated technician in a couple of weeks' time, you will get your crown made. And then you come back for it to be fitted. Well in this case, we're not doing anything. All that you're waiting for is just 20 minutes after the laser scanning, let the robot in the back make the crown for you. And once that's done, you come back for the fitting and we put it on, And the problem is solved. After an hours’ time. So here on the screen, you can see that we have one of the scans as you can see here, this is a 3D image of the area that we worked on. And you can clearly see here that we have a tooth that has a problem. And basically what I have to do is go and decide where to put the crown, tell the software that where the margins is and then design it. Once I design it. And I'm happy with it. You go to the next stage where it's the milling and then basically if I say send to mill Then we'll go there and it will start milling it for us. And that is where you have to wait about 20 minutes, and we normally ask the patient at this stage and say, well, you can simply have a rest and read something at the reception. We have a lot of inquiries about anxiety in dentistry especially in relation to implants and a lot of people don’t realize that treating an implant is exactly as any other treatment an anesthetic injection in the side of the work will numb anything from taking a tooth out, to doing an implant to doing a simple feeling. When you’re numb, you’re numb. So, that sensation of pain will go. So at the time when we do a procedure especially so with an implant. Once you have that injection and you are properly numb then you will not feel any pain for the duration of the procedure. Some people might say, well, what about the injection itself. Well, we’ve got two ways of dealing with this experience. I’ve been doing this for quite a while. A lot of people know that there’s a numbing cream. More importantly, we have actually the digital injection, which is a computer controlled injection system. So basically, that will take me from the equation and measures exactly how much pressure that’s going into the injection site. So it will determine how much pressure and how much injection to give at that time. So you’re very, very comfortable. A lot of people will think, well its that it? Yes. And from then on, all is good. So, dental microscopy. Well, since 2006 when I started just SMILE. I’ve decided to go under the microscope. So now every patient that comes in here will go under the microscope. Starting from a normal checkup to any procedure that we need to do. And the advantage of that is simple. If you see it better. You’ll do a better job. And I could simply show you now the difference between a normal tooth size and what I actually see on every day basis under the microscope. And then you will appreciate the amount of information that we see so we can provide a better service. So this now is what you see as the naked eye sees. That’s a normal tooth size. And if we can move you up to the screen now. And I’ll show you how it looks like in the frame there. So this is the least amount of magnification that we have. And you can see if I move on, I put in that step. And I normally work at 8x magnification which is that slide as you can see, the tooth is almost full enough for the whole screen. And then you can see the amount of animation that comes through. So there’s no room for mistake – if you see it better. You can do a better job. Simple as that.
